Request Contact Hours (APTA Pennsylvania Chapter Components Only)

  • Contact Hour Request Form
    • This approval form is for APTA Pennsylvania Chapter components ONLY. 
    • All external parties must submit all required information to the PA State Board of Physical Therapy.
    • Components must complete the CEU request form 90 days prior to the continuing education course (or 30 days prior for 1-2 hour District Meeting continuing education). 

The APTA Pennsylvania only pre-approves courses sponsored through and for the APTA Pennsylvania Districts/Special Interest Groups (SIGs)/Committees. See the documents below to review the process by which districts are to issue contact hours for clinical education provided at their respective district meetings. (District directors, please share the process with your education/program chairs) 

Materials Needed After Approval 

Please be sure to indicate whether the hours are general or evaluative on the sheet before printing it out to give to attendees. The district director should have the certificates to distribute to attendees. We, the professional development committee, realize that verifying membership may not be possible at the time of the meeting so we will use the honor system of identifying non-members who need to pay the $50 to get the certificate. Also, remember that you need to send the information to chapter office 30 days in advance for district meetings and 90 for continuing education seminars.
