Educational webinars are live meetings that take place on your computer and/or phone. It can be a presentation, a discussion, a demonstration or an instructional “how-to” session. Participants view a live video feed, an application or documents via computer. These are a great way to receive the education you need as a physical therapist directly related to your profession from the comfort of your home or office.

To request and advertise your meeting or webinar, please complete the following webinar forms.

Step 1: Save The Date Webinar Form

The purpose of this form is for the Committee Chairs/SIG Chairs/District Leaders to request dates and times for designated meetings or webinars. Complete this form to reserve a meeting date (it is suggested that date & speakers are confirmed prior to filling out Save The Date). This form can also serve as the only form to submit for organizers holding smaller meetings of 4 to 5 people.

Step 2: Webinar Registration Form

The purpose of this form is to provide the Chapter office information for meetings/webinars. The chair of the sponsoring SIG(s)/Committee(s)/District(s) must complete and submit this form to the chapter office – this includes post-test questions/answers, any handouts, if available.

Step 3: Webinar Invitation Form

The sponsoring SIG/Committee/District is also responsible for completing this form. This form provides the Chapter office specific information that should be included in the invitation that will be emailed to the individuals eligible for registration.

Step 4: Apply for CEs


Components must complete the CEU request form 90 days prior to the continuing education course (or 30 days prior for 1-2 hour District Meeting continuing education).
